We are seeking a skilled Case Manager to join our team at Christus Health. As a Case Manager, you will play a critical role in ensuring the efficient and cost-effective progression of patients through the continuum of care.
Key Responsibilities- Collaborate with patients, families, physicians, and interdisciplinary teams to identify and manage optimal patient flow and throughput.
- Provide comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
- Ensure continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management.
- Fulfill payer-based requests regarding individual members, where appropriate and pertinent.
- Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
- Licensed as a Registered Nurse to practice nursing in the state of New Mexico or Current licensure as an LMSW/LISW/LCSW in New Mexico.
- Case Management certification highly desirable (ACM, CCM, RN-BC).
- Two years' experience in a Health Care setting preferred, with at least one year in an Acute Care Setting.
General office environment and general hospital department/nursing units (100% of day). Ability to move around the hospital to all units/department for about 80% of the day, in office 20% of the day.
