About the Role

We are seeking a skilled LPN Field Staff to join our team at Ohio Living Home Health and Hospice. As a home health and hospice professional, you will provide high-quality care to patients in their homes, working independently and as part of a collaborative team.

Key Responsibilities
  • Provide ongoing assessment and nursing care to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Work closely with patients, families, and healthcare teams to ensure seamless care coordination
  • Maintain accurate and up-to-date patient records using mobile devices
Requirements
  • Current Ohio nursing license
  • One year of acute care or medical-surgical experience
  • Experience in a Medicare-certified home health agency and knowledge of EMR preferred
  • Valid driver's license, insurance, and acceptable driving record
