Job Summary

Centerstone is seeking a dedicated and compassionate Care Specialist to join our team. As a key member of our multidisciplinary team, you will provide person-centered assistance to clients in the development of life skills and personal goals.

Key Responsibilities
  • Coordinate client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client's clinical record.
  • Teach and train clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements.
  • Connect clients with community support systems as determined to be appropriate, and maintain regular communications with schools, OFC's and other family service providers.
  • Provide telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
Requirements
  • Associate's degree required.
  • Two years of professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
