Key Responsibilities
  • Provide primary and family evidence-based care, inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
  • Administer immunizations and injections, provide patient/parent vaccine counseling and education, and order and administer medications as needed.
  • Collaborate with other healthcare professionals to maintain a comprehensive plan of care, ensuring seamless transitions and optimal patient outcomes.
  • Develop and implement individualized care plans, taking into account patient preferences, values, and cultural backgrounds.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient care and outcomes.
Requirements
  • Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.
  • Minimum of two years of medically relevant experience or equivalent.
  • Basic Life Support (BLS) certification, including performing CPR and operating an Automated External Defibrillator (AED).
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
