Key Responsibilities:
- Deliver on-site primary care services to patients in a team-oriented environment
- Manage a panel of 200 patients, with an average of 12-16 patients per day
- Develop and implement care plans tailored to patients' unique needs
- Collaborate with clinical support teams, including LPNs, RNs, and Social Workers
- Access diagnostics, such as labs and x-rays, as needed
- Partner with community resources, including home care and hospice services
Requirements:
- Graduate of an accredited Nurse Practitioner or Physician Assistant education program
- Board certification or preparation through AANP, ANCC, or NCCPA
- Licensure through the Florida Board of Nursing or Board of Medical Practice
- Ability to obtain and maintain DEA certification for the State of Minnesota
- Excellent communication and interpersonal skills
- Valid driver's license and minimum state-required liability auto insurance
Preferred Qualifications:
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
