Responsibilities
  • Prioritize patient needs and create, implement, and assess care plans to achieve optimal outcomes.

  • Build relationships with patients, families, and staff, focusing on managing emotional and physical pain to promote healing and reduce suffering.
  • Facilitate learning for all members of the healthcare team, ensuring everyone is well-informed and up-to-date on the latest care practices.
  • Use problem-solving strategies and utilize available resources to support patients, families, and staff as part of a systems thinking approach.
Requirements
  • Completion of an accredited nursing program and a minimum of 2 years of experience in a relevant role.
  • Current Registered Nurse License and BLS certification.
  • Highly preferred: experience in hospice or home health care.
