About the Role

Aveanna Healthcare is seeking a skilled and compassionate Part Time Community Health Nurse to join our team. As a Private Duty Nurse, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Implement and manage physicians' prescribed plans of care for our patients
  • Conduct thorough patient assessments and coordinate care with our multidisciplinary team
  • Administer prescribed medications, treatments, and therapies as needed
  • Maintain accurate and timely documentation of care provided in our electronic system
  • Provide health education and training to patients and their families
  • Collaborate with our Case Managers to ensure seamless care transitions and address any concerns or changes in patient condition
Requirements
  • Active, unencumbered RN license in the state of practice
  • Current CPR certification from a recognized organization
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
