About the Role:

Elara Caring is seeking a skilled Hospice Social Worker to join our team on a PRN basis. As a Hospice Social Worker, you will have the opportunity to provide personalized care to patients in their homes, making a meaningful impact on their lives and their families.

Key Responsibilities:

  • Conduct Initial and Follow-up Social Work Evaluations: Evaluate patients' psychosocial, financial, and environmental needs through various documentation methods.
  • Develop and Implement Care Plans: Develop, verify, and update the Plan of Care, ensuring continuity and quality of care according to physician orders.
  • Provide Counseling and Support: Provide counseling and instruct patients and families on following the Plan of Care.
  • Collaborate with Healthcare Team: Participate in patient care conferences and provide supportive communication to all stakeholders.
  • Ensure Patient Safety: Implement and revise safety plans using available resources, preventing avoidable hospitalizations and ER visits.

Requirements:

  • Master's Degree in Social Work: Master's degree or Doctoral degree in social work from a social work from a school of social work accredited by the Council of Social Work Education.
  • Current State License: Current state license as a Clinical Social Worker (LiCSW or LCSW).
  • Experience in Healthcare: 1 year of social work experience in a healthcare setting.
  • Social Work Skills: Social work skills as defined as generally social work standards.
  • Valid Driver's License: Valid driver's license and insurance and reliable transportation to perform job tasks.
