Job Overview:

We are looking for a highly motivated and self-sufficient Physical Therapist to join our home health care team in Eastlake, CO. This part-time position requires traveling to patient locations in the surrounding areas.

The ideal candidate will have a current Physical Therapist (PT) licensure in Colorado, BLS CPR certification, and at least one year of experience as a PT. Home health care experience is preferred but not required.

You will be responsible for conducting initial and ongoing comprehensive assessments, providing interventions and therapeutic treatments, and collaborating with other healthcare professionals to ensure seamless patient care.
