POSITION OVERVIEW
We are looking for a committed Nurse Practitioner (NP) to become a vital part of our team on a full-time basis. The ideal candidate will possess a robust background in Physical Medicine and Rehabilitation, Neurology, or Orthopedics. We also offer training opportunities for less experienced Advanced Practice Providers (APPs). This position allows for collaboration in a supportive environment, making a significant difference in patient outcomes.
KEY RESPONSIBILITIES AND DUTIES
Responsibilities of all CommuniCare Rehabilitation Nurse Practitioners:
1. Deliver direct patient care, including assessment, diagnosis, and treatment, under the guidance of the supervising physician.
2. Work with the interdisciplinary team to create and execute personalized care plans for patients in inpatient rehabilitation and long-term care, particularly those dealing with chronic pain, orthopedic, and neurological conditions.
3. Conduct regular patient rounds and follow-up visits to assess progress and modify treatment plans as needed.4. Provide education to patients and their families regarding medical conditions, treatment alternatives, and self-care strategies.
5. Engage in clinical meetings, case discussions, and quality improvement projects to ensure the provision of high-quality, evidence-based care.
EDUCATION AND QUALIFICATIONS
Essential:
- Master's degree from an accredited Nurse Practitioner program.
- Current NP license in the state of Maryland.
- A minimum of two years of clinical experience is preferred, though not mandatory.
- Strong clinical assessment and diagnostic capabilities.
- Excellent communication, teamwork, and interpersonal skills.
- A commitment to delivering compassionate, patient-focused care.
