Correctional Healthcare Specialist

We are seeking a highly skilled and compassionate Correctional Healthcare Specialist to join our team at YesCare. As a Community Linkage Coordinator, you will play a critical role in ensuring the seamless transition of patients from correctional facilities to appropriate care settings.
Key Responsibilities- Review high-risk patient lists and determine if special needs replacement is required.
- Establish and maintain a network of community resources for special needs patients.
- Assist in obtaining necessary information related to benefits and prequalification documentation.
- Collaborate with community resources and site physicians to procure appropriate services and placement for patients.
- Coordinate with the courts, attorneys, and correctional administration to ensure prompt transition to the appropriate facility.
- Assist in planning and implementing the goals and objectives of programs and projects.
- Provide comprehensive social work to restore the individual to their maximum functioning capacity.
- Prepare documentation, records, and reports as necessary.
- Participate in and conduct in-service programs, and assist in the orientation of new staff.
