Position Overview
The role involves managing internal referrals and the intake process for patients transitioning from healthcare facilities and clinics. This position serves as a crucial link between hospitals, medical offices, home health agencies, and medical suppliers.
Key Responsibilities
- Assessment: Gather and submit patient information for evaluation regarding the suitability of homecare services. Complete necessary documentation in accordance with agency and hospital policies. 10%
- Referral and Intake: Keep homecare personnel updated on potential referrals from hospitals. Accurately process patient referrals and guide them through the appropriate home care service line. Ensure comprehensive communication with families, patients, hospital case management, physicians, and other care team members to facilitate the referral and intake process. 10%
- Initiation of Referrals: Initiate necessary referrals and specialty services. Effectively communicate and implement changes as needed. Present home health, hospice, and monitored at-home services to families, patients, and healthcare providers. Clearly explain homecare services to patients and their families, including admission criteria. 10%
- Intake Completion: Accurately complete the intake process to ensure a smooth transition from facility or hospital to home care services. 10%
- Ongoing Communication: Maintain regular communication with hospital case management and other services to ensure continuity of care. Collaborate with all staff involved in patient care, including physicians, social workers, and therapists. Serve as an educational resource for team members and liaise with physicians, hospitals, insurance companies, and medical equipment providers. 10%
- Active Participation: Engage actively as a liaison with physician offices, hospitals, and medical support services. Assist in training sessions for agency staff and community education initiatives. Strive to foster good relationships with team members and hospital staff while adhering to safety policies and recognizing inappropriate patient care referrals. 10%
- Routine Visits: Conduct regular visits and various telehealth consultations in home care services. Support medication management, delivery, and patient education. Facilitate calls to the appropriate discipline and schedule necessary clinician visits, ensuring compliance with regulatory requirements. Assist with interdisciplinary group preparations, case conference preparations, patient discharges, and record closures. 10%
- Quality Assurance: Perform quality assurance audits as requested. 10%
- Support Roles: Provide assistance to assistant managers, registered nurses, certified nursing assistants, and office staff. 10%
- Medication Administration: Assist at various facilities with medication administration and patient rounds. 10%
This position does not involve management duties and will report to a supervisor, manager, director, or executive.
Educational Requirements
Must be a graduate of an accredited nursing program.
Experience Requirements
Minimum of 1 year of experience in home care settings.
Licensure Requirements
Must hold a valid license to practice as a Licensed Practical Nurse or Registered Nurse in South Carolina, along with a South Carolina Driver's license.
Work Schedule
Weekend shifts available.
