Job Summary

We are seeking a highly skilled Nurse Care Specialist to join our nursing group. The successful candidate will provide rehabilitation and coordinate medical needs for clients, while maintaining a focus on compassion and meeting the needs of our clients in a variety of settings.

Key Responsibilities

  • Attend and actively participate in weekly team meetings to discuss client progress and address any concerns.
  • Provide medication monitoring and administration by oral medication or needle injections, as ordered by the physician.
  • Assist with keeping within assigned budget and ensure that all medications are ordered, reviewed, monitored, and discontinued as necessary.
  • Conduct assessments and approve plans of care and service delivery methods to ensure high-quality care.
  • Perform competency and skills evaluations on employees who provide direct care.
  • Inform the physician and staff of changes in the client's medical or mental condition and needs.
  • Write progress notes, summaries, and discharges to maintain accurate records.
  • Implement treatment and rehabilitative activities and strategies to support client goals.
  • Report significant incidents or accidents and notify proper authorities of circumstances or situations that present possible health hazards.
  • Follow consumer health plans and promote consumer choice and facilitate independence.
  • Assist in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures.
  • Other tasks as assigned.

Qualifications

  • Must be 21 years of age.
  • Holds a bachelor's degree in nursing or related health and human services field from an accredited college or university OR an RN from an accredited two-year program.
  • Valid driver's license and insurance required.
