Home Health RN Case Manager Role

This position offers a unique opportunity to work with a leading healthcare organization in Colorado. As a Home Health RN Case Manager, you will be responsible for providing skilled nursing care to patients in their homes, evaluating client needs, and conducting case management under the direction of a licensed physician.
Key Responsibilities:
- Develop and modify individualized plans of care based on initial and ongoing assessments.
- Implement infection control programs and educate patients and families about resources identified in the plan of care.
- Supervise CNAs and LPNs and delegate specific tasks per Nurse Practice Act.
Requirements:
- Graduate of an accredited school of nursing; BSN preferred.
- One to two years of experience in home health or hospice care.
- Current Colorado RN license and CPR certification.
