Patient Care Coordination

At Amedisys, we are seeking a skilled Clinical Case Manager to join our team. As a key member of our care team, you will be responsible for performing patient assessments and collaborating with healthcare professionals to develop and implement a comprehensive plan of care. Your expertise will be invaluable in identifying patient needs and making referrals to other disciplines as necessary. By promoting patient-centered care, you will play a critical role in ensuring our patients receive the highest quality care possible.

Key Responsibilities:
  • Conduct thorough patient assessments to identify care needs and develop individualized plans of care.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Make referrals to other disciplines as indicated by patient needs, or document rationale for not doing so.
  • Promote patient-centered care and advocate for patient needs.

Requirements:
  • Current RN licensure and relevant clinical experience.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
