About the Role:
We are seeking a skilled Healthcare Transition Specialist to join our team at UnitedHealthcare. In this role, you will play a critical part in ensuring a seamless transition of patients from institutional settings to the community.
Key Responsibilities:
- Assess and develop individualized care strategies for patients, focusing on the most appropriate, least restrictive community-based settings.
- Identify and initiate referrals for social services, including financial, psychosocial, community, and state supportive services.
- Provide intensive psychosocial rehabilitation and transition services, including Cognitive Adaptation Training (CAT).
- Complete regular screenings and assessments to ensure continuity of care.
- Communicate and coordinate with service coordinators to ensure patient needs are met.
- Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.
- Make outbound calls and receive inbound calls to assess patient health status and identify gaps or barriers in treatment plans.
- Assist with providing housing options and collecting documents.
- Conduct in-person visits and assessments to ensure patient needs are met.
Requirements:
- Bachelor's Degree in health, social services, or a related field, and 1+ years of relevant experience in assisting people in transitioning from institutional settings to the community.
- Active Social Worker license or Licensed Behavioral Health Professional in the state of Texas.
- 3+ years of experience working within the community health setting in a healthcare role.
- 1+ years of experience working with persons with a serious mental illness (SMI) and/or experience of mental illness.
- The ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications:
- Master-level Social Worker (LMSW, LCSW) or Professional Counselor, Licensed Bachelor Social Worker (LBSW), Provisionally Licensed Psychologist (PLP), or Licensed Psychological Associate (LPA) licensed to practice in Texas.
- A background in managing populations with complex medical or behavioral needs.
- 5+ years of experience working with MS Word, Excel, and Outlook.
- Case management experience.
- 5+ years of experience documenting in an electronic documentation system.
- Certified Case Manager (CCM).
- Experience in discharge planning.
- Experience with arranging community resources.
- Bilingual in Spanish and/or Vietnamese.
