Occupational Therapist

We are seeking a skilled Occupational Therapist to provide high-quality medical support to our clients in their homes. As a key member of our team, you will play a vital role in helping patients achieve their rehabilitation goals and regain independence.

Key Responsibilities:

• Assess and evaluate patients' therapeutic, rehabilitative, and functional status to develop personalized treatment plans.
• Provide occupational therapy services, including directing treatment, instructing patients and caregivers, and evaluating home environments.
• Collaborate with healthcare professionals to ensure seamless care coordination and patient outcomes.
• Identify patient and family needs for other home health services and refer as necessary.
• Prepare and submit clinical summaries and progress reports to ensure accurate patient documentation.

Requirements:

• Master's degree in Occupational Therapy from an accredited program.
• Current Occupational Therapy License in the state.
• At least two years of experience as an Occupational Therapist, preferably in community or home health settings.
• Excellent communication and organizational skills, with the ability to work independently and as part of a team.
• Current CPR certification.

Part-time, Monday to Friday, with weekend availability.

Compensation:
