Hospice Clinical Liaison

Compassus is seeking a skilled Clinical Liaison to join our team. In this role, you will be responsible for promoting the Compassus philosophy and upholding our values of Compassion, Integrity, Excellence, Teamwork, and Innovation. You will work closely with referral sources, hospitals, and facilities to identify patients who may benefit from our hospice services.

  • Excellent communication and interpersonal skills are essential for this role, as you will be working with patients, families, and healthcare professionals to provide education and support.
  • Strong assessment and problem-solving skills will enable you to identify patient needs and develop effective plans for care.
  • You will be responsible for facilitating the admission process, ensuring that all necessary forms are completed and signed, and that patients receive the care they need.
  • Collaboration with our team will be key to success in this role, as you will be working closely with admission coordinators, nurses, and other healthcare professionals to ensure seamless transitions of care.
  • A strong understanding of healthcare regulations and policies is necessary for this role, as you will be responsible for ensuring compliance with all relevant laws and guidelines.

Requirements

  • An active and unencumbered Registered Nurse license in the state(s) of employment is required.
  • A minimum of two to three years of related experience is preferred, with a strong background in hospital and/or long-term care clinical experience.
  • Bachelor's degree in Nursing is highly preferred.
