At Sunrise Senior Living, we are seeking a dedicated and compassionate RN Wellness Nurse to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to assess their health and identify any potential issues.
- Collaborate with the Resident Care Director to develop and implement individualized care plans that meet the unique needs of each resident.
- Monitor and document resident vital signs, including weight, temperature, and blood pressure.
- Provide education and support to residents and their families on health-related topics and medication management.
- Participate in quality improvement initiatives to ensure the highest level of care and service for our residents.
- Graduate of an accredited nursing program.
- Current state license as a Registered Nurse (RN).
- Minimum of one year of experience in a healthcare setting, preferably in senior care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
