About the PositionWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the homes of our patients, identifying potential health risks and developing personalized care plans to address them.
Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Work collaboratively with physicians, case managers, and other healthcare professionals to develop and implement care plans
- Provide health education and support to patients and their families
- Collaborate with primary care physicians to ensure seamless care coordination
- Travel to patients' homes and other community environments to conduct assessments and provide care
- Master's degree in nursing or a related field
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
