Patient Care Navigator

We are seeking a highly skilled Referral Coordinator to join our team at Sadler Health Center Co. As a Referral Coordinator, you will play a crucial role in managing patient referrals and coordinating care.
Key Responsibilities:- Referral Initiation and Completion: Initiate and complete referrals in a timely manner, ensuring seamless communication with patients and healthcare providers.
- Consult Note and Test Result Management: Obtain consult notes and test results from specialists, ensuring accurate and up-to-date information.
- Patient Information Verification: Verify patient demographics and insurance details to ensure accuracy and compliance.
- Clinical Information Assembly: Gather and provide necessary clinical background to specialists, facilitating informed decision-making.
- Insurance Coordination: Coordinate with insurance companies and review organizations to obtain prior approvals and ensure timely approval of services.
- Patient Communication: Review referral details and expectations with patients, ensuring clear understanding and satisfaction.
- Barrier Assessment and Resolution: Identify and assist in resolving barriers to specialty appointments, working collaboratively with Community Health Workers when necessary.
- Advocacy and Support: Advocate on behalf of patients with insurance carriers to ensure timely approval of services and resolve any issues that may arise.
- Appointment Reminders and Scheduling: Remind patients of scheduled specialist appointments via mail or phone and assist with scheduling as required.
- Prenatal Care Tracking and Reporting: Maintain and track prenatal care and delivery outcomes, providing valuable insights for quality improvement.
- Referral Coordination and Collaboration: Collaborate with outside co-locating specialists to ensure seamless care coordination.
- Productivity Tracking and Reporting: Track and report on productivity, ensuring efficient use of resources and identifying areas for improvement.
- Liaison Role and Communication: Act as a liaison between Sadler Health Center Co. and regional specialty offices, facilitating effective communication and collaboration.
- Timely Deliverables and Accuracy: Ensure all deliverables are on time and accurate, maintaining the highest standards of quality and professionalism.
- Education: High School Diploma/GED
- Certifications: Medical terminology/CPT and ICD coding course/certification
- Experience: Minimum of 6 months experience in a clinical setting using medical terminology, CPT, HCPCS, and ICD coding
- Preferred Qualifications: Previous referral experience in a multiple provider medical practice, insurance verification experience, working with an electronic health record system, and working with low-income and/or underserved or multi-ethnic populations
