Salary
We offer a competitive salary, with an estimated $40,000 student loan repayment or $30,000 sign-on bonus for eligible candidates. Additionally, you will receive $2,500 relocation expenses for those moving within 50 miles of the position location.
Job Description
This part-time (20 hours/week) role as a Healthcare Transition Specialist involves performing comprehensive assessments and documenting findings in a concise manner, adhering to documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations. Your responsibilities include ensuring accurate ICD10 coding and documentation, addressing quality elements, and conducting initial medication reviews.
Required Skills and Qualifications
To be successful in this role, you should possess:
- A Certified Nurse Practitioner or Physician Assistant through a national board
- A graduate degree in nursing (MSN) program or equivalent, with Adult-Gerontology Acute Care Nurse Practitioner certification (AG AC NP), Adult/Family or Gerontology Nurse Practitioner (ACNP)
- An active and unrestricted license in the state where you reside
- Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Access to reliable transportation for traveling to client and/or patient sites
- Ability to lift a 30-pound bag and navigate various dwelling conditions
