  • Essential Responsibilities:

  • Provide comprehensive care aligned with the medical home model, including collecting health histories, performing physical exams, conducting diagnostic tests, and prescribing medications.
  • Assess the needs of your patient population and encourage participation in annual health evaluations, health coaching, and wellness promotion activities.
  • Offer clinical leadership through participation in team meetings, clear communication, process improvement, and support for change management efforts.
