We are seeking a highly skilled and compassionate Patient-Centered Psychiatric Social Worker to join our interdisciplinary team at Kaiser Permanente. As a key member of our mental health team, you will provide direct assessment, diagnosis, treatment planning, and intervention to our patients and their families.
Key Responsibilities- Conduct thorough assessments and develop effective treatment plans to address patients' mental health needs.
- Provide individual, group, and family therapy to patients of all ages, including children, adolescents, and adults.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
- Develop and maintain strong relationships with patients, families, and community resources to promote optimal health outcomes.
- Stay up-to-date with current evidence-based practices and guidelines in mental health treatment.
- Document patient interactions and treatment plans accurately and efficiently.
- Master's degree in a related Behavioral Health Care field.
- Two years of post-graduate mental health therapy experience within the last five years.
- Licensed Marriage and Family Therapist (California) OR Licensed Professional Clinical Counselor (California) OR Licensed Clinical Social Worker (California).
- National Provider Identifier.
- Evidence-based practice experience.
