About the Role

As a Home Health Aide or Personal Care Aide, you will work collaboratively with our care team to deliver personalized care services to our clients. Your primary responsibilities will include:


  • Providing health care services in patients' residences
  • Performing domestic and household tasks
  • Transporting and accompanying patients to medical appointments
  • Reminding clients to take prescribed medications
  • Assisting with personal care activities
  • Monitoring patients' vital signs and reporting on their condition
  • Maintaining patients' care records and documenting provided services
  • Assisting patients with mobility and physical therapies/exercises
  • Instructing and counseling patients and families on diet and exercise
  • Providing companionship and basic emotional or psychological support

Requirements

To be successful in this role, you will need:


  • Proven working experience as a Home Health Aide or Personal Care Aide
  • Ability to monitor vital signs
  • Familiarity with basic nutrition and personal hygiene standards
  • Bilingual skills (English, Spanish, Creole, Russian, Mandarin)
  • A caring and compassionate personality
  • Valid HHA or PCA Certificate
