Amedisys Inc. is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care Specialist. As a key member of our patient care team, you will be responsible for coordinating and overseeing all aspects of patients' home care, ensuring they receive the highest quality care and support in the comfort of their own homes.
Responsibilities- Coordinates and oversees all aspects of patients' home care, including developing and implementing care plans in collaboration with patient care teams.
- Educates families on patient condition, needs, and care, providing them with the knowledge and support they need to make informed decisions.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications, ensuring patients receive the best possible care.
- Administers and maintains catheters, gives injections, IV therapy, or tube feedings, and completes documentation timely and accurately.
- Supervises Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), ensuring they provide high-quality care and support to patients.
- Regularly communicates patient progress to the clinical manager, physician, and care team, ensuring seamless coordination of care.
- One year of clinical experience as a Registered Nurse (RN), with a minimum of six months of home health experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification and valid driver's license, reliable transportation, and liability insurance.
- Able to assess patient status and identify requirements relative to age-specific needs.
- Familiar with general use and functions of the computer, including user names and password concepts, internet, email, navigation of computer desktop, and online HR services and training programs.
