Key Responsibilities:
  • Provide comprehensive, evidence-based care to patients with acute and chronic illnesses, focusing on prevention, early intervention, and coordinated care.
  • Lead a care team to ensure seamless patient care, including care promoters, care facilitators, and care coordinators.
  • Engage with hospitalists, case managers, and other healthcare professionals to ensure continuity of care.
  • Develop and implement individualized care plans, incorporating patient goals, preferences, and values.
  • Stay up-to-date with the latest evidence-based medicine, including lifestyle and nutrition factors that impact patient health outcomes.
Requirements:
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
  • Current, active State Medical License for the state(s) in which you'll be working.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty (preferred).
  • Excellent communication and interpersonal skills, with a focus on building trust and rapport with patients and colleagues.
  • Ability to work independently and collaboratively as part of a high-performing team.
  • Proficient in Microsoft Office Suite products, including Word, Excel, PowerPoint, and Outlook.
  • Fluency in English, verbal and written (with the possibility of additional language requirements in some centers).
