The Laurels of Hillsboro is seeking a Registered Nurse to join our team as a Nurse Care Manager. In this role, you will be responsible for planning, coordinating, and providing nursing care to our residents, as well as supervising and mentoring our team of LPNs, CNAs, and other healthcare professionals.
Responsibilities:- Develop and implement individualized care plans for residents, ensuring their unique needs and preferences are met.
- Supervise and mentor LPNs, CNAs, and other team members, providing guidance and support to ensure high-quality patient care.
- Conduct regular assessments of residents' health status, identifying and addressing any changes or concerns.
- Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in long-term care.
- Current Registered Nurse licensure in the state
- 1-3 years of experience in a long-term care setting, preferably in a leadership or supervisory role
- CPR certification
