Overview

The Outpatient Registered Nurse (RN) is an essential role within the Clinical Advancement Program (CAP), focusing on the delivery and coordination of high-quality patient care. This position involves comprehensive responsibilities including assessment, planning, intervention, and evaluation for a designated patient group. The RN will also delegate appropriate tasks to other patient care staff, such as RNs, LVN/LPNs, and Patient Care Technicians. As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, the RN engages in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient outcomes and operational efficiency.

Key Responsibilities

All duties must be executed in alignment with The Job Network's policies, procedures, and nursing standards, as well as state and federal regulations.
  • Conducts essential functions under the supervision of the Supervisor, with support from the Clinical Educator, Preceptor, or in collaboration with fellow Registered Nurses.
  • Performs systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the medical record and making necessary adjustments to treatment plans.
  • Evaluates and documents the educational needs of patients and families to provide both initial and ongoing education.
  • Collaborates with patient care staff to ensure the safe and effective delivery of hemodialysis therapy.
  • Administers prescribed medications and documents the rationale and effectiveness.
  • Initiates or assists in emergency response measures as required.
  • Acts as a resource for the healthcare team and participates in training and orientation for new staff.
  • Ensures proper laboratory collection and processing procedures are followed, rescheduling any missed collections.
  • Identifies expected outcomes and updates nursing assessments and care plans in collaboration with the Interdisciplinary Team.
  • Educates patients about transplant and treatment modality options.
  • Completes CAP requirements for advancement.
  • Performs additional duties as assigned by the Supervisor.

Physical Requirements and Work Environment

  • This role involves direct patient care, which may require heavy lifting and assistance with patient mobility.
  • Frequent prolonged periods of standing are necessary, and the employee must be able to bend over.
  • Occasional movement of equipment weighing up to 200 lbs. may be required, along with lifting solutions weighing up to 30 lbs.
  • The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels.
  • Exposure to infectious materials may occur.
  • Daily tasks include desk work, computer usage, and interaction with patients and healthcare staff.
  • Travel to training sites or other facilities may be necessary.
  • Employees may be required to perform essential functions in various locations with similar physical demands and working conditions.
  • Completion of the Ishihara's Color Blindness test is required as a condition of employment. Note that failing this test does not automatically disqualify a candidate; reasonable accommodations will be considered.

Supervisory Responsibilities

Oversight of Patient Care Technicians/LPNs/LVNs/RNs may be assigned as a Team Leader or designated Nurse in Charge, contingent upon:
  • Successful completion of all required education and training for new employees.
  • A minimum of 9 months of experience as a Registered Nurse.
  • A minimum of 3 months of experience in chronic/acute hemodialysis as a Registered Nurse.

Education and Licensure Requirements

  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Must meet practice requirements in the state of employment.

Experience and Skills Required

  • Entry-level position for RNs with less than 1 year of Nephrology Nursing experience in the last two years.
  • Preferred minimum of 9 months experience as a Registered Nurse.
  • Preferred experience in chronic/acute hemodialysis.
