We are seeking a qualified Licensed Practical Nurse (LPN) to join our team at Aspire Health Group. The successful candidate will provide direct patient care to residents and patients in our skilled nursing facility, assisting them with activities of daily living under the direction of a professional nurse.
Key Responsibilities- Implement Individualized Treatment Plans: Assist in the development and implementation of individualized treatment plans for each assigned resident, ensuring their unique needs are met.
- Supervise Certified Nursing Assistants (CNAs): Supervise and evaluate the performance of CNAs, providing guidance and support to ensure high-quality care.
- Oversee Falls and Pressure Sore Programs: Oversee and implement programs to prevent falls and pressure sores, ensuring the safety and well-being of residents.
- Provide Nursing Services: Provide nursing services to residents, including administering medications, wound care, and other medical interventions as needed.
- Conduct Resident Evaluations: Conduct thorough evaluations of each resident's medical status upon admission and throughout their stay, documenting findings and recommendations.
- Current LPN License: Possess a current, unencumbered, active state license to practice as an LPN.
- Experience in Skilled Nursing/Rehabilitation: Preferably, have experience working in skilled nursing or rehabilitation facilities.
- Qualifications and Skills: Be qualified, compassionate, and dedicated to providing high-quality care to residents.
