Responsibilities- Evaluation and Treatment Planning: Conduct thorough evaluations to develop personalized treatment plans that cater to the unique needs of our patients.
- Collaboration and Communication: Work closely with our multidisciplinary team, including physicians, therapists, and family members, to ensure seamless care coordination and enhance patient outcomes.
- Discharge Planning: Perform discharge evaluations to make recommendations for home and community safety, and provide referral and education resources to support a smooth transition.
- Documentation and Reporting: Maintain accurate and detailed records of patient progress, including treatment plans, evaluations, and discharge summaries.
- Education: Bachelor's degree in Occupational Therapy from an accredited program.
- Licensure: Current, unrestricted license as an Occupational Therapist in the state where practicing.
- Certifications: Current CPR Certification.
- Experience: Minimum of one year's work experience as an Occupational Therapist.
- Skills: Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses, effective communication and organizational skills, and attention to detail.
