We are seeking a compassionate and skilled RN Case Manager to join our team at Heart of Hospice, LLC. As a Hospice RN, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.

About the Role

As a Hospice RN Case Manager, you will be responsible for:

  • Conducting initial nursing evaluations to determine eligibility for hospice services
  • Identifying patients' physical, psychosocial, and environmental needs
  • Developing and implementing individualized plans of care
  • Collaborating with patients, families, and healthcare teams to ensure seamless care delivery
Requirements

To be successful in this role, you will need:

  • One year of clinical experience as a registered nurse
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Access to a dependable vehicle or public transportation
