Key Responsibilities- Develop and implement outreach programs to engage patients and improve health outcomes.
- Collaborate with healthcare teams to identify and resolve complex issues related to patient care.
- Build and maintain relationships with physicians, staff, labs, and vendors to ensure smooth coordination of efforts.
- Develop and maintain spreadsheets and/or databases to track patient contact activity and monitor program progress.
- Coordinate and perform outbound communication processes through various channels, including phone, email, and mail.
- Assist with creating annual outreach work plans and maintain schedule tasks.
- Monitor accuracy and completeness of data collected and recommend improvements to data collection tools and processes.
- Bachelor's degree or equivalent experience.
- Ability to be highly persuasive and to effectively convince patients to commit to prescribed actions.
- Demonstrated ability to openly and comfortably discuss sensitive topics with patients of both genders.
- Able to build positive relationships with a variety of clients.
- Detail-oriented and able to handle multiple projects simultaneously.
- Ability to handle a large volume of inbound and outbound telephone calls for a variety of clients.
- Written and verbal communication skills.
- Proficient in Microsoft software products, including Word and Excel.
This is a full-time position, working 40 hours per week, Monday through Friday. Occasional weekend work may be required.
