Your expertise will span Family Medicine, Internal Medicine, Pediatrics, and Women's Health settings, emphasizing preventative care, disease management, and health promotion.
This role requires you to work collaboratively with a supervising physician, providing longitudinal care and managing chronic illnesses. You will also be responsible for coordinating staffing and orientation of assigned team members, planning ongoing education and training, and maintaining infection control/safety standards.
Key Responsibilities- Provide assessment, diagnosis, treatment, education, referral, and follow-up for patients in a shared or owned panel.
- Manage multiple chronic illnesses, perform minimally invasive procedures, and work with a stable patient population.
- Recommend screenings and tests based on patients' demographics.
- Manage healthcare within protocols mutually agreed upon by physician and APP team members.
