Key Responsibilities:

  • Deliver nursing care to residents in post-acute settings
  • Adhere to and follow resident care plans diligently
  • Measure and monitor vital signs, reporting findings to supervisors as needed
  • Observe residents' conditions during shifts and communicate any concerns to the nursing team
  • Assist with meal service and support residents during feeding
  • Help residents with daily living activities (ADLs) such as personal hygiene, mobility, and toileting
  • Maintain cleanliness by sanitizing surfaces, changing linens, and ensuring a hygienic environment
  • Act as a liaison between medical staff and residents, facilitating communication with visitors
  • Assist in moving residents in and out of beds and wheelchairs, ensuring their comfort
  • Provide companionship and a compassionate presence to residents

Qualifications:

  • Current certification as a Certified Nursing Assistant (CNA)
