- Collaborate with healthcare teams to develop and implement effective care plans that address the physical, emotional, and social needs of patients.
- Provide education and support to patients and their families on mental health topics, including coping strategies, stress management, and community resources.
- Develop and maintain relationships with community agencies and organizations to ensure seamless transitions of care and access to resources.
- Participate in multidisciplinary teams to develop and implement policies and procedures that promote patient-centered care.
- Stay up-to-date with best practices in clinical social work and mental health treatment to ensure high-quality care.
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Massachusetts.
- At least 5 years of experience in clinical social work, with a focus on mental health and patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong assessment and treatment planning skills, with the ability to develop and implement effective care plans.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
