Job Description

The Quality Coordinator will be responsible for coordinating clinical care needs for patients related to HEDIS and other quality initiatives. This role will conduct in-home Point of Care testing for care gap closure and enter data into our database to capture and reconcile POC tests performed.


Essential Duties and Responsibilities:
  • Retrieve and review data collected to confirm gaps in care are captured.
  • Combine data from all systems for accurate precise reporting.
  • Perform general nursing assessment and utilize results to determine additional patient needs in collaboration with the RN Case Management team.
  • Educate patients on procedures being scheduled/performed and reason for performing them.

Requirements:
  • High school diploma or GED equivalent required.
  • 2+ years' experience in a healthcare environment.
  • Active CNA license in applicable states.
  • Recent and related experience working with HEDIS/STARS program.
