The Hospice Registered Nurse (RN) plays a vital role in delivering compassionate care to patients in their homes, assisted living facilities, and skilled nursing environments. Reporting directly to the Clinical Care Manager, the Hospice RN is dedicated to enhancing the quality of life for patients and their families through effective assessment, intervention, and education.
Work Schedule: Monday to Friday, 8 AM - 5 PM
Qualifications:- Must hold a valid nursing license in the State of Florida.
- Preferred: At least one year of nursing experience in a hospital, home health, or hospice setting.
- Demonstrates strong ethical standards and maintains a professional demeanor.
- Possesses critical thinking skills, effective time management, and technical proficiency.
- Ideal candidates are organized, self-motivated, and collaborative team players.
- Reliable transportation is required, along with proof of automobile insurance and a valid Florida driver's license.
- Manage and assess the needs of patients and their families in the community.
- Conduct ongoing evaluations of the physical, psychological, social, and spiritual needs of patients.
- Supervise and support LPNs, CNAs, and other team members in the field.
- Identify and address potential issues proactively.
- Foster positive relationships with interdisciplinary team members.
- Educate the community about hospice care and its philosophy.
- Maintain regular communication with the Clinical Care Manager regarding patient care and risk management.
- Develop and implement nursing care plans, ensuring they are regularly reviewed and updated.
- Adhere to professional nursing standards and complete required documentation efficiently.
- Participate actively in interdisciplinary team meetings to discuss patient needs.
- Stay current with necessary skills to provide optimal patient care.
- Ability to manage multiple tasks while adapting to changing priorities.
- Ensure compliance with patient care standards and documentation regulations.
- Assist in maintaining compliance with federal and state regulations.
- Proficient in using Electronic Medical Records.
