Adult BHH Care Coordinator

Overview

Spurwink is seeking a skilled and compassionate Adult BHH Care Coordinator to join our interdisciplinary team. As a key member of our team, you will work closely with clients, their families, and healthcare providers to develop and implement individualized care plans that address the complex needs of adults with behavioral health challenges.

Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans that integrate behavioral and physical health needs and goals.
  • Facilitate team meetings and collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination.
  • Provide ongoing support and guidance to clients and their families, addressing physical and behavioral health-related risk factors and connecting them with community resources.
  • Monitor and document progress, ensuring timely and effective interventions to meet client needs.
  • Participate in individual and group supervision, required trainings, and case consultations to maintain expertise and stay up-to-date on best practices.

Requirements

  • MHRT/C or MHRT/C eligible.
  • Bachelor's degree in a related field preferred.

What We Offer

  • A competitive hourly rate of $21.00 to $26.00, depending on experience.
  • A $5000 sign-on bonus, paid in two installments: $2500 after 3 months and $2500 after 6 months of full-time employment.
