Position Overview:
We are in search of a dedicated and empathetic Practical Nursing Specialist (LPN) to become a vital part of our supportive housing initiative. The LPN will be instrumental in delivering top-notch healthcare services to our residents, collaborating closely with a multidisciplinary team to ensure holistic care and support.
Key Responsibilities:
- Deliver direct nursing care to residents, including administering medications, providing wound care, and monitoring vital signs.
- Conduct comprehensive health evaluations and collaborate with the healthcare team to create personalized care plans.
- Observe residents for any changes in health status and respond effectively to emergencies and urgent health issues.
- Assist in coordinating care, including scheduling medical appointments and facilitating communication with healthcare professionals.
- Educate residents on their health conditions, treatment options, and medication management.
- Maintain precise and timely documentation of all nursing care provided and interactions with residents.
- Support residents in their recovery by promoting healthy lifestyle choices and encouraging adherence to treatment plans.
- Participate in multidisciplinary team meetings and contribute to case management discussions.
- Provide emotional support and crisis intervention as necessary.
- Uphold the highest standards of ethical and professional conduct in all interactions with residents, staff, and external partners.
Minimum Qualifications:
- Graduation from an accredited Practical Nursing program; plus two years of experience as a Licensed Practical Nurse.
Certificates and Licenses Required:
- Current license or multistate licensure privilege to practice as a Licensed Practical Nurse (LPN) in Virginia;
- Valid motor vehicle driver's license;
- MANDT certification (within 3 months of hire);
- CPR certification (within 3 months of hire);
Necessary Special Requirement:
- The selected candidate will be required to undergo a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.
Additional Work Schedule Requirements:
- This position operates on a shift schedule that may include evening hours.
- This role is classified as emergency/essential services personnel, requiring reporting for duty during inclement weather and other emergencies.
Preferred Qualifications:
- At least 2 years of experience managing care for individuals with serious mental health issues and co-occurring substance use disorders.
- At least 2 years of experience in a medical-surgical environment.
- Demonstrated strong clinical skills with the ability to perform nursing duties accurately and independently.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.
- Proven ability to work effectively as part of a multidisciplinary team.
- Knowledge of and commitment to trauma-informed care practices.
Physical Requirements:
- Ability to sit, walk, stand, bend, cook, shop, drive, and write as needed; utilize a keyboard for data entry in electronic health records and review data on a computer monitor.
- Capability to take vital signs (blood pressure, weight, abdominal girth, temperature, pulse, blood sugar finger sticks) and administer intramuscular injections and TB tests.
- Must be able to provide nursing care and assistance, including physically assisting large adults with hygiene tasks. All duties performed with or without reasonable accommodations.
