As an expert in care delivery systems, the CNC assists patients and families in navigating the intricate healthcare landscape, advocating for their needs while collaborating with healthcare providers across the unit and the broader system.
The CNC is responsible for enhancing care delivery within a distinctive healthcare environment, with a core emphasis on ensuring safe, high-quality, evidence-based care within complex systems.Key Responsibilities
1. **Leadership and Development**
- Inspire, motivate, and drive change while taking charge of professional growth and development.
- Model and embody the Leadership Pledge consistently and proactively.
- Step into the role of Nurse Manager during their absence.
2. **Strategic Alignment**
- Align departmental objectives with the strategic vision of Excela Health, implementing initiatives that support the mission and values.
- Propose at least one process improvement initiative annually.
3. **Quality Assurance**
- Engage in root cause analysis for recurring issues and take necessary actions.
- Ensure compliance with safety, environmental, and infection control standards.
4. **Communication and Collaboration**
- Facilitate open communication within the team and maintain a department communication board.
- Participate in daily meetings to ensure alignment and address patient care needs.
5. **Clinical Oversight**
- Supervise daily clinical activities to guarantee adherence to the PA Nurse Practice Act and hospital protocols.
- Conduct patient rounds to assess care quality and staff compliance with nursing standards.
6. **Patient Education and Coordination**
- Develop educational processes to enhance patient care activities, promoting health and self-care.
- Ensure patients and families receive comprehensive education tailored to their clinical needs.
7. **Safety and Compliance**
- Uphold patient safety protocols in accordance with hospital policies.
- Engage patients and families in safety processes to minimize risks.
Qualifications
- Graduate of an accredited Nursing Program.
- Minimum of one year of relevant nursing experience.
- Strong leadership, organizational, and critical thinking skills.
- Current licensure as a Registered Nurse in Pennsylvania required.
- Basic Life Support (BLS) certification from an approved provider.
