Key Responsibilities- Case Management: Conduct thorough assessments of family strengths and needs, identifying problem areas and developing comprehensive treatment plans to effect growth and change.
- Treatment Planning: Participate in the development, implementation, and ongoing evaluation of treatment plans, ensuring the best possible outcomes for youth and families.
- Alternative Placement: Develop and implement alternative placement or permanency resources for youth who may not return to their families, ensuring a smooth transition to a stable and supportive environment.
- Case Management Services: Deliver effective case management services, including scheduling counseling sessions, ensuring children receive required medical and dental examinations, and tracking and monitoring services provided.
- Confidentiality and Compliance: Ensure compliance with confidentiality standards, parental consent, and necessary release of information, maintaining the highest level of professionalism and integrity.
- Casework Contacts: Conduct regular casework contacts with families, engaging youth and family members in treatment goals, and ensuring children are maintained in safe and secure environments.
- Interagency Collaboration: Act as a bridge between youth and family, and foster care treatment teams, facilitating communication and collaboration to achieve optimal outcomes.
- Reunification Support: Assist with reunification of youth with family, community, and school, implementing permanent supports and resources, including mentorship, after-school recreational and job opportunities, and relapse prevention programs.
- Education: Minimum Bachelor's degree in human services or a related field.
- Experience: Prefer 1 year of experience working in the child welfare system.
- Flexibility: Must be able to work a flexible schedule with variable hours, including day, evening, weekend, holiday, and subject to call hours.
- Transportation: Must have a valid driver's license and reliable vehicle, with the ability to coordinate and plan travel for weekly visits and appointments.
- Cultural Competence: Ability to work with economically and culturally diverse populations, demonstrating sensitivity and understanding of diverse backgrounds and experiences.
- Skills: Ability to handle crisis intervention in a solution-focused manner, as well as strong communication, written, verbal, and listening skills, with experience in customer service, writing, and engagement.
Minimums and maximums may vary based on skills, experience, and other relevant factors.
- $25.24/hour ($52,500/annually) - Bachelors degree + 0 years of relevant experience
- $26.20/hour ($54,500 annually) - Bachelors degree + 2 years of relevant experience
