We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality care to our residents in a supportive environment. The LPN will be responsible for administering medications, monitoring patient vital signs, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:

  • Provide direct patient care under the supervision of a registered nurse or physician.
  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor and record patient vital signs and medical information accurately.
  • Assist in developing and implementing care plans for residents.
  • Collaborate with other healthcare professionals to ensure comprehensive care for residents.
  • Maintain a clean and safe environment for residents and staff.
  • Communicate effectively with residents, families, and team members.
  • Participate in ongoing education and training to stay current with nursing practices and regulations.
  • Adhere to facility policies and procedures, as well as state and federal regulations governing nursing care.

Qualifications:

  • Valid LPN license in the state of Illinois.
  • CPR certification.
  • Minimum of 1 year of experience in a long-term care or skilled nursing facility preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively in a fast-paced environment.
  • Demonstrated compassion and empathy towards residents and their families.
  • Proficient computer skills for documentation purposes.
  • Ability to work rotating shifts, including weekends and holidays.
