Job Overview

Alliance Health System is actively seeking to enhance our team with skilled professionals. This position offers a unique opportunity to contribute to a progressive organization dedicated to improving health outcomes for our community.

We are in search of a Patient Care Coordinator who will serve as a crucial link between our healthcare providers and patients. This role involves overseeing treatment plans, facilitating connections between patients and healthcare professionals, and assessing patient progress. Success in this position requires a proactive approach to patient consultation and collaboration with providers to ensure effective interventions.

Key Responsibilities:

  • Establish and maintain strong relationships with both patients and healthcare providers.
  • Conduct regular meetings with assigned providers to review the active patient roster, ensuring timely updates and modifications to enhance patient care.
  • Monitor patient adherence by reviewing daily schedules, delegating necessary adjustments to scheduling staff, and following up to ensure completion.
  • Coordinate necessary imaging for patients as required.
  • Work closely with the Medical Team to arrange physician appointments as needed.
  • Demonstrate comprehensive knowledge of Alliance Health System's services and technological offerings.

Qualifications:

  • Exceptional verbal and written communication abilities.
  • Strong analytical skills with a focus on solution-oriented approaches.
  • Outstanding customer service capabilities.
  • Empathy and compassion in patient interactions.

Preferred Skills:

  • Bachelor's degree in Healthcare Administration or a related field is preferred but not mandatory.
  • Prior experience in a similar healthcare setting, such as Sports Medicine, Chiropractic, Physical Therapy, Pain Management, or Orthopedic Surgery.
