About the Role

As a Home Health LPN with CenterWell Home Health, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing personalized, comprehensive care that addresses the unique needs of each patient.

Key Responsibilities
  • Provide skilled nursing interventions to patients in their homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and outcomes in a timely and accurate manner, using our electronic health record system to track and analyze data.
  • Participate in ongoing education and training to stay current on best practices and industry developments, ensuring that our patients receive the highest level of care.

Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
