Job Title: Physical Therapist

Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment at a great Skilled Nursing Facility (SNF) / Long Term Care (LTC) in Santa Cruz, California.

Job Summary:

The Physical Therapist will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and developing a treatment plan to improve their physical function and mobility. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong passion for providing high-quality patient care.

Responsibilities:
  • Assess patients' medical conditions and develop a treatment plan to improve their physical function and mobility.
  • Provide direct patient care, including physical therapy interventions and education.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Document patient progress and maintain accurate records.
  • Stay up-to-date with the latest physical therapy techniques and best practices.
Requirements:
  • Graduation from an accredited physical therapy program.
  • Current state license as a Registered Physical Therapist or actively pursuing licensure.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
