Home Hospice Nurse

We are seeking a skilled Registered Nurse to join our team of professionals who share a passion for delivering exceptional patient-centered care in the comfort of patients' homes. Palliative Care Specialist Responsibilities:

• Assist in the planning, implementation, and coordination of patient care services to ensure seamless delivery of compassionate care.
• Collaborate with interdisciplinary teams to address patients' physical, emotional, and spiritual needs.
• Document patient progress and outcomes to inform care decisions and improve quality of life.

Requirements:

• Current New York State Registered Nurse License/Permit
• At least two (2) years of experience in nursing, including medical-surgical, oncological, palliative, and community home health/hospice settings
• Preferred qualifications: one (1) year of community hospice or home health experience, NYS driver's license, and BSN from an accredited school

Salary Information:

The estimated salary for this position is $54.65 - $68.71 per hour, based on location and experience.
