Cardiology Partner Job Summary

Sage Health - North Little Rock Market, AR is seeking a skilled Cardiology Partner to join our team-based care model. As a key member of our care team, you will work closely with physicians, advanced practice professionals, and other healthcare experts to provide high-quality care to our patients.

Primary Responsibilities:
  • Evaluate and treat patients with cardiovascular diseases, including heart failure, coronary artery disease, and other conditions.
  • Coordinate patient services, including specialty referrals, cardiology subspecialists, and vascular surgeons.
  • Participate in the care team and provide medical expertise to ensure comprehensive care.
  • Collaborate with primary care physicians and nurse practitioners to provide coordinated care.
  • Document patient information accurately and efficiently.
  • Follow established policies and protocols.
  • Meet with primary care physicians and market medical director to discuss quality of care, outcome data, and medical records issues.
  • Focus on direct patient care, including patient-facing time and administrative tasks.
  • Support our longitudinal care model for heart failure, disease management, and prevention.
  • Contribute to readmission reduction initiatives for heart failure and CAD.
  • Develop a framework for upstream stroke prevention.
  • Coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
  • Interpret and document test results, including echocardiograms and stress tests.
  • Coordinate patient care with cardiology subspecialists.
Required Qualifications:
  • Graduate of an accredited MD or DO program.
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
  • Board Certification in Cardiology.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • Excellent interpersonal and communication skills.
  • Familiarity with team-based care models.
  • Ability to adapt to a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email, EMR, and other software.
Preferred Qualifications:
  • Knowledge of HEDIS/STARS.
  • Knowledge of CAHPS and HOS.
  • Experience working in medically underserved/culturally diverse communities.
  • Experience working with electronic health records.
  • Experience working in Medicare Advantage Global Risk Models.

No call, no weekend, and no pediatrics.

This role is considered patient-facing. If selected, you will be required to undergo TB screening.
