Unit Specific Information

The Children's Health Center CRS is the only multi-specialty interdisciplinary clinic in Northern Arizona, designed to optimize patient outcomes. Our extensive services include:

  • 12 pediatric specialty services.
  • Physical therapy.
  • Occupational therapy.
  • Speech therapy.
  • Registered dietician services.
  • Social work support.
  • Audiology services.
  • Comprehensive case management.
Additional Information

We also provide various services to assist with wheelchair evaluations, medical equipment dispensing, hearing aids, feeding therapy, and dental screenings in collaboration with local educational institutions. Please note that travelers will be scheduled four months in advance due to credentialing and insurance requirements. An NPI number will be necessary for billing purposes, with the application process managed through the facility.
