POSITION OVERVIEW

The Rehabilitation Therapist will implement a tailored, comprehensive physical therapy program that includes assessment, treatment, planning, execution, education, and communication to enhance a patient's journey towards achieving functional objectives.

Services are delivered in a healthcare setting, playing a crucial role in preventive care, rehabilitation, and the management of patients with chronic conditions, illnesses, or injuries. The Rehabilitation Therapist will also foster a collaborative working relationship with other healthcare professionals and support staff. The goal is to meet or exceed established productivity benchmarks.

KEY RESPONSIBILITIES

Core Functions:
Delivers direct patient care, assesses functional requirements and outcomes, and collaborates with other specialists. Reviews patient needs and modifies care plans to ensure optimal results.

Creates and executes a care plan for patients based on a comprehensive assessment of potential development, involving significant others when appropriate.

Conducts therapy practices within established care standards. Upholds professional and ethical standards.

Supervises and guides support staff to achieve patient functional goals while adhering to state practice regulations and payer requirements.

Communicates effectively with other professionals and support staff to ensure positive patient outcomes.

Educates patients and their families about the recovery process and how to navigate challenges throughout.

Adheres to facility and departmental policies, objectives, quality improvement initiatives, safety protocols, and infection control standards.
Accurately documents evaluations, treatment objectives, and daily treatment plans promptly. Regularly updates documentation and maintains prescriptions and signatures in compliance with physician orders.
Promotes professional growth and development for self and others through participation in educational programs, in-service meetings, and workshops. Stays informed on current professional literature.
Provides leadership and direction to staff and serves as a resource to promote physical therapy within the community. Conducts in-services as needed to enhance care.
Maintains a thorough understanding of various funding sources and requirements to ensure service reimbursement.
Travels between facilities as necessary to provide patient care.
Offers in-person or telehealth services as appropriate.
Adheres to infection control policies and practices of MJ Care and the assigned facility.

GENERAL DUTIES

Delivers services in accordance with departmental guiding principles.
Maintains strict confidentiality of information and records at all times.

Demonstrates commitment to ongoing learning, including personal professional development and sharing knowledge with team members.

Safely transports and transfers patients as necessary, following proper policies and procedures.
In emergencies, ensures safe transport of individuals per disaster protocols.
Maintains compliance with TB and flu requirements for MJ Care and assigned facilities.

QUALIFICATIONS
Education
Degree in Physical Therapy from an accredited institution recognized by the American Physical Therapy Association.
Current state license and/or certification to provide physical therapy services in good standing.
Valid National Provider Identifier (NPI).
Experience
Preferred 1+ year of experience.
Skills and Knowledge
Proficient in computer applications including Microsoft Office. Ability to effectively utilize electronic medical records systems.
Strong communication skills, including concentration, ability to learn, and active listening.
Excellent problem-solving, organizational, time management, and mathematical skills.
Ability to interact professionally with internal and external customers at all levels.

Knowledge and skills to address the physical and developmental needs of the patient population receiving services.

Ability to remain composed in emergency or crisis situations and provide appropriate assistance.

Valid driver's license in the state of residence, acceptable driving record, reliable vehicle, and proof of valid auto insurance as per state regulations.


LEVEL I ACCESS

Safeguards access to Personal Health Information specific to assigned patients for screenings, consultations, evaluations, mandatory site-specific meetings, chart audits, payment, healthcare operations, and therapy.

PHYSICAL REQUIREMENTS & WORKING CONDITIONS

While performing the duties of this role, the associate is frequently required to communicate effectively; stand and walk; bend and/or kneel; squat, balance, and/or crawl; reach above shoulder level; push, pull, and/or twist up to 100lbs; navigate stairs; lift and/or carry up to 100lbs and occasionally more than 100lbs; and utilize fine motor skills.

Specific vision abilities required for this role include the ability to read documents, view computer screens, and observe the workspace.

Specific hearing abilities required include the capacity to hear in a moderately loud work environment (open area with multiple conversations).

This position includes exposure to biohazards (i.e., bloodborne pathogens). The work environment may involve limited space with equipment or exposure to equipment not suitable for pacemakers. The role requires the use of protective equipment, including but not limited to gowns, gloves, and face protection.
